  • How do I register as a new patient?

Our team will do the best we can to match you with a physician that meets your preferences. Our new patient online registration form will be available soon!
  • What are your clinic locations and hours?

  • What languages are spoken?

  • Are your clinics wheelchair accessible?
  • Yes, our clinics are all wheelchair accessible. A key priority of the Health Clinic by Shoppers™ is to ensure accessibility for all Canadians.

  • How do I schedule an appointment?
  • Please use our online booking system to book an appointment. You can also call the clinic directly for more information and to book an appointment – please see the Clinic Locations & Details page for contact details.
  • How do I cancel or reschedule an appointment?
  • If you booked your appointment using our online booking system, you can reschedule/cancel your appointment through your online account. Otherwise, please call the clinic to reschedule or cancel your appointment. Please visit the Clinic Locations & Details page for contact details.
  • SERVICES
  • Are all services at your clinic covered by the provincial healthcare system?
  • Most routine medical services are covered by your provincial healthcare system. To find out more about a specific service and cost that may not be covered by your provincial healthcare system, please call your clinic. Please refer to our Clinic Locations & Details page for contact details.
  • If I am not covered by my provincial health system, may I still come in and receive care?
  • Yes! Individuals not covered by their provincial health system may still receive care at our clinics however, you may be charged for such services.
  • What payment methods are available?
  • We accept major credit cards (VISA and MasterCard), debit card, and cash.
  • What other forms of insurance do you accept?
  • We accept out of province provincial health insurance, Workplace Safety & Insurance Board (WSIB), and Interim Federal Health Program (IHFP) insurance.   • Can I still see a provider if I have misplaced my provincial health card?
  • A valid provincial health card is required for check-in at every visit. Please visit Service Ontario for more information on how to replace a lost or stolen health card in Ontario.

    Please note that for Ontario residents, as of July 1st, 2020, all red and white health cards will no longer be accepted as proof of coverage for Ontario Health Insurance Plan (OHIP) services. Please visit serviceontario.ca/redandwhite for more information on how to obtain a new photo health card.

    You may still see a healthcare provider at the clinic without a valid provincial health card however, you may be charged for such services. Please call your clinic for further information.
  • Can I have my prescriptions renewed over the phone?
  • Yes! However, there may be a charge for this service. Please call your clinic for further information.
  • How do I obtain a copy of my medical records?
  • At this time, please call the clinic directly at 416-256-9642 to inquire about obtaining a copy of your medical records. Please note that depending on the scope of your request, you may be charged an administrative charge for this service. Please call your clinic for further information.
  • YOUR PRIVACY
  • How do you safeguard my personal health information?
  • We will only collect, use, and disclose your personal health information with your consent or as required by law. For more information on our privacy practices, please see our Privacy Policy.

    Please do not send any personal or confidential health information to our clinic email addresses. To book an appointment, please use our online booking tool or call the clinic during our regular business hours. Emails will be returned the next business day.
  • Who has access to my personal health information?
  • We use an electronic health record to share your personal health information with our healthcare team so that you get the best care possible, no matter which clinic location you attend. However, you can limit this access for healthcare purposes by asking to have a Lockbox applied to your patient record. Please see our Lockbox Guide for more information. If you would like to proceed with restricting access to your electronic health record then please contact your clinic for more information.
  • How do I obtain a copy of my medical records?
  • At this time, please call your clinic directly to inquire about obtaining a copy of your medical records. Please note you may be charged an administrative fee for this service. Please call your clinic for further information.
