The Health Clinic by Shoppers™ is made up of healthcare professionals who work together to deliver convenient access to consistent, high-quality primary care services. The Health Clinic by Shoppers™ is owned and operated by Shoppers Drug Mart Inc., which is a wholly owned subsidiary of Loblaw Companies Limited.

Protecting your privacy

The Health Clinic by Shoppers™ is committed to protecting the confidentiality of your personal health information. It is our policy not to collect, use or share personal health information without the patient’s consent, unless permitted or required by law. Personal health information includes your name, address, phone number, health care number, date of birth and information regarding your health, including but not limited to medical condition, medical history and the care that you received.

We collect your personal health information directly from you or from the person that is legally acting on your behalf. Occasionally, we collect personal health information about you from other sources if we have obtained your consent to do so or if the law permits.

The Health Clinic by Shoppers™ uses an electronic health record to share your personal health information with our healthcare team so that you get the best care possible, no matter which Health Clinic location you visit. We will use and share your personal information:

  • to provide you with treatment and care
  • to communicate or consult about your healthcare with other healthcare providers
  • to get payment for your treatment and care including from OHIP, WSIB and private insurance
  • to plan, administer and manage our internal operations
  • for risk management activities
  • for quality improvement activities (such as sending patient satisfaction surveys)
  • for teaching
  • for research
  • to compile statistics
  • to comply with legal and regulatory requirements, and
  • to fulfill other purposes permitted or required by law.

We will request your express consent for any other uses of your personal information not listed above. In the course of providing services or maintaining our systems, we may use service providers who may require access to systems which contain personal health information. These service providers are bound to keep all personal health information confidential.

Shared Health Information System

The Health Clinic by Shoppers™ participates in the exchange of personal health information through two provincial shared health information systems: Ontario Laboratories Information System (OLIS) and Ontario’s Health Report Manager (HRM). Consistent with the Personal Health Information Protection Act, also known as PHIPA, these shared systems permit approved health care providers to securely access and use electronic patient health information. This access ensures the delivery of more timely and patient-centered care.


When you provide us with your personal health information we assume that you understand that the information will be used and/or shared with others as described above.

You can also limit access to your personal health information for healthcare purposes by asking for a consent directive, also known as a lockbox. There are several kinds of consent directives. You can lockbox your entire record, a specific visit, or staff member at one of the Health Clinic by Shoppersᵀᴹ locations from seeing your record. To request a consent directive be added to your record, please contact your healthcare provider.

We may not be able to fulfill your wishes if they impact our ability to deliver quality health care to you or if we are legislated to use or disclose the information to which you object. In these cases, we will discuss the impact of your objection with you.

Security Safeguards

The Health Clinic by Shoppers™ strives to maintain appropriate physical, procedural and technical safeguards so as to protect against loss, unauthorized access, disclosure, inappropriate alteration or misuse of personal health information.


You have the right to know what personal health information is held in your file and have the right to question and request correction of any erroneous information. To access your file, a written request should be sent to your healthcare provider. Please note, there are administrative fees associated with providing access to health records.

Retention and Storage

Your personal health information is retained for as long as necessary to fulfill the purpose for which it was collected and to comply with applicable laws.

Our Policy, Inquiries or Concerns

As Shoppers Drug Mart is an independent operating division of Loblaw Companies Limited, the Health Clinic by Shoppers™ is subject to the Loblaw Privacy Policy.
